This June, Dr. Alberto Herreros de Tejada participated in the LXXXI Congress of the Spanish Society of Digestive Pathology with their presentation «RME, DSE and Polypectomy. Complications, frequency and resolution«;
«Cold snare polypectomy is the recommended basic technique for the treatment of sessile polyps ≤ 10 mm, with a high safety profile. Hot snare polypectomy allows for the removal of pedunculated polyps larger than 10 mm. The use of mechanical prophylaxis with stalk ligation or adrenaline injection is indicated for lesions ≥20 mm or those with a stalk greater than 10 mm.
RME in its various guises permits the approach of non-pedunculated lesions. ESD allows treatment of extensive lesions with suspected superficial submucosal invasion or in the presence of fibrosis.
Bleeding and perforation are the main complications associated with the techniques described, along with post-coagulation syndrome. We must correctly identify their risk factors and effective preventive measures. In situ treatment should be based on experience and appropriate instrumentation. We must work as a team with our colleagues and with the other specialties involved in managing complications (Surgery, Radiology, etc…).
